Unity Population Health and the Next-generation Vaccine Platform

The COVID-19 pandemic has highlighted how important it is for primary care organizations to remain flexible and seek health IT solutions that can help manage large patient populations—even with limited time and resources.

As healthcare technologies continue evolving, and the clinical data spectrum continues expanding, health platforms must have the capability to combine multiple technological functions and create automation workflows without changes to the health organization’s existing proven processes.

Unity Population Health Management (PHM) achieves just that with the integration with Allscripts® Practice Management and Allscripts Professional EHR™. Unity’s offerings include custom modules that help identify, automate and optimize various processes in a health center’s Care Management & Operations spectrums, and helps improve team productivity without changing current processes.

All the modules are customizable to the unique needs of the organization and can be implemented independently to improve specific areas such as: clinical compliance, care management, remote patient monitoring, claims optimization, and more.

Vaccine Module

The COVID-19 vaccine roll out has been widely successful in certain areas, but not everywhere. This will potentially lead to avoidable bottlenecks and even portions of the population going without a vaccine for longer than necessary. Unity Health has developed a vaccination module to help health centers manage vaccine scheduling and administration for large volumes. It enables organizations to seamlessly track and manage patients throughout all phases of the vaccination process with integration into Allscripts Practice Management.

The dynamic scheduler enables healthcare organizations to open up appointment slots in real time based on vaccine availability and admin capacity. The paperless system enables patients to schedule, re-schedule, provide consent, answer pre-screening questionnaires, and capture and send a picture of the insurance card directly from their mobile devices through a link provided via text message.

The iPad-based vaccinator role app helps vaccine screeners and administrators complete the check-in, take consent, authorize and complete vaccination documentation as required by regulation.

Does this address only the covid-19 vaccine or is it all vaccines?
