14-Hospital Health System Automates Patient Quality Management and Discharge Process, and Provides Electronic Health Record for California Home Health Network

St. Joseph Health System (SJHS), a $4.6 billion not-for-profit Catholic healthcare system based in Orange, CA that provides a broad range of medical services in California, West Texas and Eastern New Mexico, has selected Care Management(TM) and Homecare(TM) solutions from Allscripts (Nasdaq: MDRX) to streamline patient movement through their 14 hospitals and enhance patient care following discharge.


The Allscripts Care Management solution will help ensure continuity of care by automating and consolidating everyday tasks for SJHS’s hospital case managers including utilization management, discharge planning, documentation integrity, audit management and quality management. The fully-integrated, Software-as-a-Service (SaaS) application also will streamline the referral of patients requiring long-term care to SJHS’s network of post-acute organizations as well as the Allscripts proprietary network of 110,000 post-acute providers.

SJHS Home Care Network Automates with Electronic Health Record

The St. Joseph Home Health Network’s home health and hospice agencies across California and Texas will deploy the Allscripts Homecare solution, providing billing, scheduling, and an Electronic Health Record in a unified, seamless electronic system. During home visits, SJHS nurses will use the EHR to document patient care and compare their assessments from previous visits. They can also generate customized care plans and OASIS quality reports for Medicare.

SJHS’s homecare agencies also will use the Allscripts Referral Management(TM) solution to review and respond to electronic referrals from SJHS and other hospitals, manage other tasks involved in the intake process, and ensure they quickly capture referred patients best suited for the care they provide.

SJHS currently utilizes the Allscripts Electronic Health Record to automate and connect its Foundation Physicians in California, physicians of SJHS’s Covenant Medical Group in Lubbock, Texas, and is providing Electronic Health Records in a community program to private practiced physicians affiliated with all of its hospitals.
