Allscripts Discharge Planning (DP) enables
hospitals to fully automate and streamline this process and connect
automatically with the nation’s post-acute care community -
cutting length of stay, while increasing throughput, reimbursement
and staff efficiency. Interfaced with the hospital’s ADT system,
the tool guides the case manager through the discharge planning process
and then sends a detailed electronic referral to the relevant facilities
in Allscripts’ proprietary database of 90,000 post-acute care
providers. First replies typically are returned within minutes.

DP facilitates the discharge planning process.
- Automatically communicates user-selected patient data and referral
information to any extended care provider
(i.e., SNF, Rehab, Home Health, DME, Dialysis, etc.)
- Transfers discharge information to providers electronically and
securely without the need for a fax machine
- Guides case managers to discharge Medicare patients on the optimal
discharge day
- Alerts online extended-care providers via pager, e-mail, and/or
fax when they receive a hospital referral and alerts care managers
when on-line providers respond to a referral
- Assigns patients to a discharge planner by location, service,
physician, patient type, financial class, or combination
- Provides tracking, reporting tools and cross-departmental collaboration
to overturn denied reimbursements that do occur
- Allows creation of electronic copies of paper-based documents
