Benefits Realization
The Benefits Realization service provides a comparative analysis of pre and post install data metrics for the discharge planning and utilization management processes with preliminary test data at 3 months that is repeated 6 months post go-live once onsite processes have stabilized. Additional analysis can be preformed as a second level review and comparative analysis of the defined metrics at nine and 12 months post go-live. Practice recommendations for additional process improvement to the discharge planning process at each facility will be included based on the data findings.

Allscripts provides a comprehensive assessment and evaluation of current case management practices. Led by clinicians with extensive experience driving measurable change through process automation, particular focus is directed to current case management processes, admission assessment, discharge planning, and the ability of case management staff to facilitate and drive the care of the patient through the continuum of services.
